Align Services provides case management services for those on the Community Choice or Development Disabilities Waiver.
Community Choice Information
The Community Choice and Developmental Disabilities Waiver services are provided in Albany, Campbell, Carbon, Converse, Fremont, Goshen, Hot Springs, Johnson, Laramie, Natrona, Niobrara, Platte, Sweetwater and Washakie Counties. The Community Choice and DD Waiver services provides support for those 65 and older, or 18 and older and blind or disabled, who require nursing home level of care but would prefer to live at home or in an assisted living facility.
Align Services provides case management services for those on the Community Choice or DD Waiver. This includes assistance with the Medicaid Application, finding and establishing services in your community, and monitoring services and overall well being with a monthly or quarterly home visit.
