Case Managers assist individuals with the entire waiver process, beginning to end.
Starting with the waiver application, once you have chosen your case manager, they will provide Targeted Case Management to help with Medicaid eligibility, psychological evaluations, and completing the ICAP (Iventory for Client and Agency Planning) – all items required to determine waiver eligibility.
Once on the waitlist, your case manager can assist in connecting you with local resources and social services to help fill gaps until waiver funding is available and a Plan of Care is developed.
Once on the waiver, your case manager will monitor your Plan of Care by completing the required home visits and service observations, ensuring wants and needs are being met, monitoring overall health and wellness, and keeping communication transparent and frequent with providers, families, and others within an individual’s circle of support.
